Door County 4-H Horse and Pony Shows and Events
Below are new forms that need to be completed by every participant (youth and adults) for shows and events put on by Door County 4-H.
- Door County 4-H Horse & Pony Registration Form
- Assumption of Risk and Photo Release … if you are planning to take photos to be used for anything (Facebook, advertising future shows, etc.), then this form needs to be filled out by everyone.
- Concussion Waiver – there are separate forms for youth and adults:
